Once you have an Evolution Health policy with Morgan Price our claims procedure is simple. All you need to do is follow the steps below:
Medical Emergency Claims
1. Where possible you should contact our helpline within 48 hours so they can assist you and arrange payment etc.
3. Select option 2 - A treatment guarantee form will not be required for emergency cases.
Non-Emergency Claims
All claims should be submitted in line with the claims procedure in your policy wording but as a summary:
IN-PATIENT CLAIMS

OUT-PATIENT & DENTAL CLAIMS
1. Visit your physician or dentist in the usual way, remembering to take a claim form with you and request the physician/dentist to complete page 3 of the form.

2. Pay for the treatment yourself – make sure you get a receipt.
3. Complete the full claim form and send it to us together with all supporting information, original invoices and/or receipts, to euroclaims@morgan-price.eu or to the postal address stated on the form. Remember to keep a copy for your records.
A bank details form can also be downloaded if required.
